CE marking of medical devices is far more than a formal step before entering the European market.
It represents the official recognition that a product meets the General Safety and Performance Requirements (GSPR) and demonstrates its clinical benefit in accordance with the European Medical Device Regulation (MDR, EU 2017/745).

Understanding CE marking of medical devices

CE marking is a mandatory prerequisite for marketing any medical device within the European Union.

For devices classified as IIa, IIb, or III, evaluation by a notified body is required.
This organization reviews:

  • The Quality Management System (QMS),
  • The technical documentation, and

Device classification under the MDR

Before initiating the CE marking process, manufacturers must determine their device’s risk class, as defined in Annex VIII of the MDR.
Classification depends on invasiveness, duration of use, and medical purpose:

  • Class I – Low risk
    Examples: dressings, corrective glasses, reusable surgical instruments.
    The manufacturer can self-assess conformity, except for sterile or measuring devices.
  • Class IIa – Moderate risk
    Examples: syringes, contact lenses, non-invasive imaging systems.
    Evaluation by a notified body is required for both technical documentation and QMS.
  • Class IIb – High risk
    Examples: ventilators, infusion pumps, orthopedic implants.
    A detailed design and manufacturing assessment is performed by the notified body.
  • Class III – Critical risk
    Examples: heart valves, pacemakers, life-support systems.
    Full review of design, clinical evaluation, and conformity assessment.

Each step up in classification increases regulatory requirements, particularly concerning clinical evidence and post-market surveillance.

Strengthened clinical requirements: the heart of CE marking

Under Article 61 of the MDR, manufacturers must prove through a clinical evaluation that the device is safe, effective, and performs as intended.
This process includes three main stages:

  1. Developing a Clinical Evaluation Plan (CEP),
  2. Collecting and analyzing relevant clinical data,
  3. Drafting a Clinical Evaluation Report (CER) validated by the notified body.

For innovative or high-risk devices, data must be obtained through clinical investigations conducted according to ISO 14155, which defines Good Clinical Practice for medical devices.

Key challenges in clinical development for CE marking

Manufacturers face several challenges when developing clinical evidence for CE marking:

  • Robust, representative data: clinical proof must reflect patient diversity and be free from bias.
  • Comprehensive documentation: all evidence must be traceable and included in the technical file.
  • Ongoing compliance: clinical conformity is not a one-time step but must be maintained throughout the device lifecycle.

A structured and demanding certification process

Obtaining CE marking for medical devices involves several structured stages:

  1. Classify the device according to MDR Annex VIII,
  2. Implement a quality system compliant with ISO 13485,
  3. Compile complete technical documentation,
  4. Undergo conformity assessment by a notified body,
  5. Affix the CE marking,
  6. Ensure ongoing post-market surveillance (PMS / PMCF).

Post-market clinical follow-up (PMCF)

Receiving the CE marking does not end clinical monitoring obligations.
Manufacturers must continue collecting and analyzing real-world data through a Post-Market Clinical Follow-up (PMCF) plan, as described in Annex XIV of the MDR.

The objective is to confirm the device’s safety, performance, and clinical benefit over time.
This can involve:

  • Post-market or observational studies,
  • Real-world evidence (RWE) from hospital registries,
  • Updated literature reviews,
  • Analysis of field data (incidents, complaints, vigilance).

Findings are summarized in a PMCF Evaluation Report, regularly updated to maintain compliance and reinforce clinical confidence.

CE marking for medical devices is no longer just a regulatory obligation—it is a symbol of clinical reliability and transparency.
By strengthening evaluation and post-market surveillance requirements, the European Union places patient safety at the center of medical innovation.

  • Reduced clinical and operational risks,
